This is what I was told to send to Delta Dental for cancellation in April 2022. I received an email reply with CANCEL in the subject line. I assumed that the insurance had been cancelled. However, Delta Dental continued to take the premium payments out of my checking account. We are due a refund for a year of premium payments.
